WHO MAY AVAIL OF THE SERVICE:
1) Must be a Filipino Citizen and a Resident of Oroquieta City age 60 yrs. Old and above;
2) A Resident of the household being applied for
3) Male or female concessionaire
4) A concessionaire with a consumption of 30 cubic meter and below
5) Only a single account is granted, regardless of the number of senior citizens residing therein.
6) Account should be under the applicant’s name.
SUBSTANTIAL REQUIREMENTS (Proof of Age and Citizenship):
1) Picture of Senior Citizen
2) Photocopy of any of the following valid I.D.’s (SSS, PRC License, Driver’s License, Voter’s ID, Philhealth ID, Postal ID)
3) Proof of Residency (Barangay Clearance issued within 2 months from the date of application)
4) Authorization Letter ( If applied through representative)
5) Photocopy of any Government issued ID of the representative
NOTE: Senior Citizens discount must be renewed annually.
